Description of the problem	2006 jeep grand cherokee after starting and putting into to drive the vehicle revved and took off with out the gas pedal being depressed. This has happened three times, with the last incident leading to a severe crash and injury. After the crash and the car stopping the engine was still revving. The incident was witnessed twice by the same person aside from the driver.
