Description of the problem	On 9/23/19 parked the vehicle. Shut off vehicle while still in drive. The vehicle did not alert me that the car was not in park. Got out of car and door locked like everything was okay - no other beeps or notifications. Vehicle rolled out of driveway and hit a tree causing significant rear damage (thankfully did not hit any other vehicle or pedestrian). Brought to honda dealership and they could not find anything although i explained that the warning indicators work intermittently. Finally on 10/2/19 i videotaped an incident where it did not alert me in anyway when i shut the car off and left in the drive gear (images from video attached). Honda states emergency parking brake should always be engaged; however, this does not fix the fact that the safety notifications are not working as expected. Definitely a vehicle safety issue.
